We know you have a choice of Healthcare providers. We know how important it is to you to trust that your physician is someone who gets to know you, your medical history and will be there to assist you and your family when you need medical care. We currently have over 30 physicians in 2 locations providing compassionate, prompt, quality healthcare to you and your family.
CHECK IN
Please arrive 15 minutes prior to your scheduled check in time. This will ensure that you are checked in and ready to be seen at your scheduled time. If you are running late, please call 823-1010 and let one of our receptionists know. Thank you for your help in keeping the schedules running smoothly.
If you arrive 15 minutes after your scheduled appointment time there is a chance that you will need to reschedule.
MEDICATIONS
Please bring a list of all the medications that you are taking to each appointment. This is important so that your physician has the most recent information about any medication you may be taking.

INSURANCE CARDS
Please remember to bring your insurance card with you to every visit. We will need to make a copy of it at each visit to verify eligibility.
Same day appointments are available with Nurse Practitioner Oglesbay, who joined us in May of 2011 and Nurse Practitioner Jamo who joined us in July of 2011. They are practicing at our Westside location.
As Nurse Practitioners board certified in Pediatrics, they can provide complete and holistic care to newborns and children up to 18 years of age. They are able to provide the same basic care that physicians provide, such as performing routine well child checks, diagnosing and treating acute illnesses, managing chronic illnesses, and addressing physical injuries and behavioral concerns.
